Post-operative Surgical Margin Positive Cases in Bladder Cancer: Radical or Partial Cystectomy

Abstract
Radical cystectomy with pelvic lymph node dissection is a standard treatment for locally advanced and high grade invasive bladder cancer. The impact of surgical margin status has not been well recognized as a prognostic variable for bladder cancer after radical cystectomy. The positive surgical margin ranged from 2% to 21% and is associated with mortality in majority of patients. Techniques that minimize the risk of positive surgical margins are likely to correlate directly with improved oncologic outcomes of bladder cancer. Every effort must be made to avoid a surgical positive margin. When it is recognized preoperatively and postoperatively that positive surgical margin is likely to occur, systemic chemotherapy might be helpful to clinical progression, disease specific survival and the rate of positive surgical margins.
